Based Services
Windsor Place At-Home Care is a licensed home health agency. Area offices include Coffeyville, Independence, Kansas City, Chanute, Columbus, Topeka, and Lawrence. The mission of Windsor Place At-Home Care is to provide independent living support services and supports for persons so that they remain in the comfort and security of their own home.
Who is eligible to receive services? Any disabled or elderly person who requires assistance to maintain an At-Home lifestyle is eligible. A referral from a social service agency is the most frequently used approach to initiate service. However, anyone may request an assessment.
Who determines what services are rendered? A Service Plan for each client detailing the tasks to be done is carefully developed by a case manager. The Service Plan is specifically designed to meet the unique needs of each individual client. This Service Plan, along with a schedule of when the services are to be rendered, is then executed by At-Home Care staff. This individualized plan is reviewed and updated as needed by the client and case manager.
What services are provided? Our qualified staff will provide assistance with:
Activities of Daily Living – Bathing, grooming and toileting, transferring, meal preparation and clean up, and mobility assistance.
Health Management Activities – Monitoring of vital signs, medication assistance, and reporting changes in function or condition to the case manager or family.
Home Management – Shopping, house cleaning, meal preparation and clean up, laundry, and life management activities such as bill paying and paperwork.
Who will pay for the cost of these services? Services are billed on an hourly basis. Clients may also purchase services on a self pay basis. If the client has long term care insurance, this could be of help. Those eligible for Medicaid assistance through the Home and Community Based Long Term Care Services (HCBS) program may receive services as approved by the State of Kansas. The At-Home Care office staff may assist with any insurance billing or Medicaid application.
How is quality of service monitored? At-Home Care is licensed by the State of Kansas as a Home Care Agency. The home care attendant’s supervisor will contact each client to confirm the quality of the service rendered to determine client satisfaction. The client may also contact the supervisor at any time for a home supervisory visit. These procedures insure the highest possible quality of care given to each client.
Windsor Place At-Home Care is also a leading provider of Financial Management Services (FMS) under the Home and Community Based Service waiver for consumers choosing to utilize self-directed options. 100% of the state reimbursement rate for Direct Service Workers goes to the benefit of the caregiver. Windsor Place At-Home Care pays its caregivers every week through direct deposit and has never had a delayed or deferred payroll since it began services in 1996!
